Great News!

Member Liaisons can now email forms to the Programming Department. Follow the instructions below to communicate with us in this fashion or feel free to continue faxing us forms as well.
• Click on the appropriate form above
• Forms will open as locked word files
• Save form as a document on your computer
• Use the tab key to get to the next field or click on each field to enter text
• Complete the form and save it again
• Open a new email and attach the saved form as an electronic document
• Send the email to mbryan@art-reach.org
• The Programming Department will confirm that the request is received within 24 hours via email
• If this confirmation of receipt is not received please call the Programming Department at 215-568-2115 x4 to confirm that the email was sent successfully


Please note: Receipt of confirmation does not confirm event availability until the official "Ticket Confirmation Sheet"is received. In addition, forms will not be processed nor will tickets be held unless fields are filled in correctly in their entirety.
