Great News!
Member Liaisons can email forms to the Programming Department. Follow the instructions below to communicate with us in this fashion or feel free to continue faxing us forms.
- Click on the appropriate form above
- Forms will open as locked word files
- Save form as a document on your computer
- Use the tab key to get to the next field or click on each field to enter text
- Complete the form and save it again
- Open a new email and attach the saved form as an electronic document
- Send the email to rrobbins@art-reach.org
The Programming Department will confirm that the request is received within 24 hours via email
- If this confirmation of receipt is not received please call the Programming Department at 215-568-2115 x5 to confirm that the email was sent successfully
Please note: Receipt of confirmation does not confirm event availability until the official "Ticket Confirmation Sheet"is received. In addition, forms will not be processed nor will tickets be held unless fields are filled in correctly in their entirety.

Attention Liaisons...Please Share Your Clients' Opinions!
The opinions of clients help Art-Reach shape programming and ticketed events. Surveys should be completed after each event. For questions please call 215-568-2115, so that an Art-Reach team member may help.
- Choose the survey tool that works best with your clients' abilities and complete the tool with them or ask them to complete it and return it to you
- Each individual attending an event should complete his/her own survey. i.e. if 5 clients attend, 5 surveys should be completed and returned
- Return the survey to Art-Reach via email, fax or postal mail (instructions for each are below)

To RSVP for this event:
- Participants must be current Art-Reach Liaisons who have attended and completed an orientation session
- Participants must complete and return a registration form by February 16, 2010
- Participants must be affiliated with an Art-Reach member in good standing and must have submitted yearly 2010 member dues, or have preparations to pay dues communicated with Art-Reach.
